This form is one of the required forms that must be sent to the individual judgment debtor once the judgment creditor or receiver has notice that property has been frozen or seized under a turnover order, order appointing a turnover receiver, a writ of garnishment, writ of execution, or other postjudgment order. It is to be used in conjunction with forms 27-34 and 27-35 in this chapter. Tex. R. Civ. P. 679b. The judgment creditor or receiver must fill in the case style and “interested persons to notify.”
