The Teacher Retirement System of Texas has promulgated the following model qualified domestic relations order for retirees. It may be downloaded at www.trs.texas.gov/Pages/QDRO-divorce-domestic-relations-order.aspx. An order for active members, which is reproduced as form 25-13 in this manual, may also be downloaded at that website.

Although using a model qualified domestic relations order as a fill-in-the-blank form is generally not advisable, TRS requires the use of their model and other versions will be rejected.
Checking with the retirement system to obtain any available current model forms and instructions is recommended.
Note that the model form contains sensitive data. See section 6 of the Introduction in volume 1 of this manual concerning protection of sensitive data in filed records.
