The caption of this request should contain the style and cause number of the underlying case. A clerk’s record is not required but may be used with a petition for writ of mandamus. The relator may either attach certified or sworn copies of relevant documents or refer to clerk’s and reporter’s records. Tex. R. App. P. 52.3(g), (h).
