The caption of this request should contain the style and cause number of the underlying case. A clerk’s record is not required but may be used with a petition for writ of mandamus. The relator may either attach certified or sworn copies of relevant documents or refer to clerk’s and reporter’s records. Tex. R. App. P. 52.3(g), (h). If a reporter’s record is needed, see form 26-19.
