Certificate of Mailing of Homestead Affidavit as Release of Child Support Lien
[Name of affiant], Affiant, appeared in person before me today and stated under oath:
“My name is [name of affiant].
“On [date], Affiant caused a Homestead Affidavit as Release of Child Support Lien to be recorded in [recording information of child support lien] (“Affidavit”).
“On [date], Affiant sent a letter and a copy of the Affidavit, notifying the claimant under the child support lien of Affiant’s homestead claim and the filing of the Affidavit, by registered or certified mail, return receipt requested, to the claimant at the claimant’s last known address.
“Attached to this certificate are a true and correct copy of the letter and proof of mailing of the letter.”
