The form of an affidavit for business records is addressed in rule 902(10) of the Texas Rules of Evidence. An unsworn declaration may be used in lieu of that affidavit. See section 8.58 of the practice notes. The business records and the declaration must be served on each other party to the case at least fourteen days before the trial begins. See Tex. R.
